  2. Download the fillable registration form below in Step 2.
  3. Open both forms and fill them out right on your computer. (You’ll need to install Adobe Acrobat if you don’t already have it. Click here to install it)
  4. After filling out both forms, save it and upload both filled out forms below in Step 3.
  5. Upload the other required documents needed to complete your enrollment packet in Step 3 as well.
